Patient Portals

Our primary care and specialist practice patient portals allow patients to conveniently access lab results, request appointments or prescription refills, and contact their physician from their computer or smartphone.

Patient Portal Transition

Beginning October 24, 2026, our patient portals will be retired as we transition to eRecord and MyChart on November 1, 2026.

After October 24, you will no longer be able to use this portal to request prescription refills, message your provider or review past information.

Need your medical records?
Through January 3, 2027, you may visit the Medical Records Department Monday–Friday, 8:30 a.m.–4:30 p.m., or call 315-787-4120. Beginning November 1, you may also call 585-275-2605. Starting January 4, 2027, in-person requests will no longer be available.

Primary Care or Specialist Practice’s Patient Portals

Our primary care and specialist practices utilize a separate portal than the hospitals. Please note you must request a portal activation letter from your physician’s office before you can access the patient portal. Please contact your physician’s office for an activation letter. If you are experiencing problems or need help with the patient portal, please contact the practice directly.

If your practice isn't listed, it is already on MyChart.

Hospital Patient Portal

The hospital patient portal for Geneva General Hospital and Soldiers and Sailors Memorial Hospital will be retired December 31, 2026 as we transition to MyChart.
