Medical Records

In compliance with New York State Department of Health regulations, all patients have the right to review their medical record without charge, or to obtain a copy of their medical record for which the hospital can charge a reasonable fee.

In order to receive a copy of your medical records you can visit the Medical Records Department, Monday through Friday 8:30 – 4:30 pm or contact us at 315-787-4120.
