Swing Bed Program
Swing Beds are utilized to provide patients with care following an acute care hospitalization, when the patient needs more time to recover from a surgery, illness or injury. The goal is for the Swing Bed patient to gain independence and prepare for transition home or to another appropriate level of care.
Patients may need a Swing Bed after a surgery, after joint repair or replacement, after a prolonged hospitalization due to an injury or illness, when IV antibiotic therapy is needed, or other reasons.
If a patient has had three overnights in an acute care bed at any facility and has an ongoing diagnosis and needs that require skilled care, as defined by Medicare, he or she may be a candidate for a Swing Bed.
Finger Lakes Health is providing the Swing Bed Program on the 1stacute unit at Soldiers & Sailors Memorial Hospital. It includes an individualized care plan designed specifically for the patient in a comfortable setting, including a private room, skilled nursing care, physical therapy, occupational therapy, speech therapy, care management, respiratory therapy, nutritional counseling, activities and compassionate care.
