Short-Term Rehabilitation

The Transitional Care Program offers a comprehensive rehabilitation approach designed to improve the functional independence of patients who have had hip/knee replacement, fractured bones, amputation, a cardiovascular accident or who have an extended medical hospitalization for pneumonia, congestive heart failure or heart ailments. The program compliments the continuum of services offered by Finger Lakes Health's Rehabilitation Services. 

TCP provides care at the Living Center at Geneva - South, 45 Mason Street, Geneva. Skilled Nursing beds and a modern rehabilitation room are dedicated to the Transitional Care Program.

The Transitional Care Program offers 24-hour care by a team of healthcare professionals made up of physicians, nurses, physical therapists, occupational therapists, speech language pathologists, social workers, certified nurses' assistants, activities and dietary personnel. The TCP is short-term placement that is designed to meet the rehabilitative and physical needs of patients/residents while they are "transitioning" to another level of care or discharge to home.

Patients admitted to this program are provided with either a semi-private or private room accommodation. The Living Center's first floor offers patients easy access to the patio area, a  bright spacious dining room, and a wide variety of leisure activities. Laundry, meal service, housekeeping and dental services are provided at no additional charge. In addition, standard cable and local telephone service is available, for an additional fee. Some rooms offer piped-in wall oxygen as needed.

Short-Term rehabilitation services are also offered at Huntington Living Center, Waterloo, NY, at The Homestead, Penn Yan, NY and the Swing Bed program at Soldiers & Sailors Memorial Hospital in Penn Yan, NY.

The team of therapists provides rehabilitative services in the same building at each of the facilities. The therapists are supervised by a physical therapist that is a board certified clinical specialist in geriatric physical therapy.

Many insurance plans, including Medicare, Medicaid, Medicare Blue Choice and others, will pay for this service. Some insurances require prior authorizations.
