A vital first step in taking charge of your health is choosing the right primary care partner and making the most of this relationship working jointly toward your happy and healthy future. Your primary care physician plays a key role in coordinating all aspects of your care. He or she can manage a variety of medical concerns. Your primary care provider, also sometimes referred to as your ”PCP,” provides preventive care and advice, maintains your medical history and can refer you to specialists when needed. Sometimes this is referred to as your “medical home.” A strong relationship with your PCP ensures continuity of care, often leads to goal setting and collaboration to achieve the best outcomes, and is critical to you feeling informed, satisfied and in control of your own health.

Finger Lakes Health provides comprehensive primary care services in Ontario, Seneca, Wayne and Yates Counties. Our practices are staffed by physicians, nurse practitioners and physician assistants, who specialize in Family Medicine or Internal Medicine. They provide comprehensive care with ready access to laboratory, radiology and specialty care. Most primary care physicians are medical doctors trained in internal medicine or family practitioners or some have earned a Doctor of Osteopathy.

To have your medical records transferred from another physician's office to a Finger Lakes Health practice please fill out the Request for Medical Records form and submit it with a copy of your driver's license to your new physician's office.
