Finger Lakes Health's Clyde Family Health Center is located at 4 West Genesee Street, Clyde, New York 14433. Care is provided by board-certified physician Dr. Sixto Medina, well-respected and experienced nurse practitioners Christina Gray, MSN, ANP, CNS and Stephanie Humbert, MSN, FNP-BC.

Clyde Family Health Center is currently accepting new patients ages 18 and older.  Insurances accepted include Blue Choice, Blue Cross / Blue Shield, Empire, GHI, Medicaid, Medicare, MVP and Fidelis.  Many other insurances and self-pay accepted. Payments can be made by cash, check or credit card.

We are open Monday - Friday from 7:30 a.m. - 4:00 p.m.

Appointments can be made by calling the Clyde Family Health Center at (315) 923-3640. 

Registered patients, can visit our patient portal. If you are not registered for the patient portal, please contact the practice for activation instructions. You can send a message to your provider, request an appointment, request a prescription refill and more on the patient portal. All patients need to contact the practice for a new login.

For after hours or weekend NON-EMERGENCY CARE please visit our Geneva Urgent Care or Seneca Falls Urgent Care.

Additionally at this site, Finger Lakes Health operates a Lab Draw Station.  All patients are welcome, regardless of the ordering physician.  Lab services will be reopening January 3, 2023. For Lab Services at this location, please call (315) 923-3641.
