Care Management

The Care Management Department is comprised of health care professionals who can assist you throughout the continuum of care. The Care Management office is located on 2 North with individuals assigned to each unit for your convenience and service. The department can be reached by dialing hospital extension 4180.

The Care Management team is committed to providing a safe, smooth transition from the hospital to home.  This profession is familiar with various community agencies and eligibility requirements, and can connect you with appropriate resources for desired community services.  Collaborating with your physician, the care manager will assist in the coordination of services to meet your discharge needs.  Direct your questions or concerns regarding discharge plans, maintaining independence at home, or alternative living arrangements to your care manager.

The Care Management Department offers the following services:

Social Work Evaluation - This evaluation assesses the social, emotional, and environmental factors that may affect one's health.  The assessment is often helpful to the physician in developing an appropriate treatment plan.

Information and Referral - The care manager maintains a working knowledge of community resources available to patients and their families. The department also provides Health Care Proxy forms and education concerning completion.

In addition, two special programs are offered through Care Management at Geneva General Hospital: Lifeline and Mobile Meals. Information can be obtained upon request.

Lifeline - Lifeline is a personal emergency response system that links you to 24-hour emergency assistance at the push of a button. Lifeline users are individuals of all ages who benefit from knowing that help is just moments away. Customers include the elderly, new parents, disabled or handicapped individuals, those recovering from surgery, those with cardiac conditions or diabetes, or bedridden individuals.

Mobile Meals - The Mobile Meals Program offers two meals a day which are delivered to your home Monday through Friday. Each day you will receive one hot meal and one cold meal. Your physician will indicate a diet specific to your needs. The Mobile Meals Program can service customers within a 2.5 mile radius of Geneva General Hospital.  To obtain the current cost for meals, please contact the Care Management Department at (315) 787-4180.
